Narrative

Employee was removing a chock block from a chock block holder and felt a pop in his wrist. He was seen by a physician on 9/4/07 and returned to work. He continued to have problems and saw an orthopedic doctor on 9/17/07. On 9/24/07 we received the diagnosis of a bone contusion and ligament sprain requiring a cast to be applied.
