Correction/Entered incorrectly: North 6, 6 entry, between 15 & 16 block, MMU 004. 18' wide X 15' long X 6' high. Support: 6' 3/4"" resin bolts, pans, screen, bars and jacks. Fall dangered off, additional roof support installed around fall. Clean-up plan developed and posted at fall site.
