A roof fall occurred in the # 9 seam, in 18 north panel, just inby the intersection of crosscut # 40 and ending just outby the intersection of crosscut # 47. The fall measured approximately 365'x18'x5'. The area was supported with 5' fully grouted rebar bolts and 9' mega bolts. An undetected slip was determined to be the cause of fall.
