Narrative

Employee's ankle was caught between the input shaft and pressure plate attempting to change a torque converter on rail locomotive in the yard. Cause of this injury was safety awarness and having help. Compliance with rules/regs not a factor, miner proficiency a factor, PPE not a factor, mining equip/systems could be a factor.
