## Narrative

The employee was an oiler and appeared to have his grease gun in hand while coming down a 9 foot permanent ladder.  He fell from between 0 and 9 feet onto a concrete pad.  The employee suffered a laceration above his left eye, and broken left lower forearm, and a break to the left side of his hip.
