Narrative

Employee was lifting lagging for arches and felt pain in groin. This accident has been under investigation since employee reported next day & reported strain groin which became a hip strain. We received a letter on 10/19/07 from the treating physician explaining how this was related. Please see attached two pages. **Employee started losing time on 9/21/2007.**
