Narrative

On August 3, 2007 the employee was performing normal work as a bagging operator. The employee was not feeling well and later that evening went to the emergency room, and was later taken to SLC for further care. During follow-up visits on 10/8/07, it was determined that the employee had an alergic reaction to metabisulfite.
