Narrative

Employee was in the process of installing a rope sheave on a 6""x40"" shaft. Shaft was stood on end. When the crane went to position the sheave it bumped the shaft causing it to tip over. The employee tried to keep the shaft from tipping over and tore a bicep muscle in his left arm.
