An unintentional roof fall occurred on the main north track, #4 entry, inby #2 portal at 129+ 78 ft. The fall occurred at a 4-way intersection. Fall was 20' x 20' x 7 feet high. Area was mined in July, 1994. Area was supported with 8 ft long point anchor roof bolts (anchor shell & 2' resin) with wooden planks & hammer blocks. No injuries occurred. Material passage was blocked.
