Narrative

On 7/24/07 employee stepped up on shield hauler forks that were at an angle, slipped in oil that was on forks, and twisted his right knee. Employee continued working until 9/18/07 when he sought medical attention and began missing work. On 10/22/07 information was received on work-relatedness of this absence.
