Narrative

The injured employee was holding the wear plate on the forks about three inches off the ground. The wear plate shifted causing wear plate to tilt forward pinching employee's three finger between the wear plate and the ground. Employee received a cut to right middle finger and was sent to the doctor and received 4 stitches
