Narrative

On 10/24/07 an unintentional roof fall occurred on the 4th South Sub Main, entry #6, at cross cut # 24. The fall was 55' long x 18' wide x 12' high. The intersection was bolted with 10' point anchor roof bolts. The area was fractured lamanated rock.
