Narrative

Our examiner and an MSHA inspector found a roof fall during a weekly examination of a worked out area. The fall was located in the 1 left, 1 right, 3 left, 4 West panel in entry #7, between crosscuts 8 and 9. The fall was adjacent to the exaimers route of travel.
