Narrative

Employee claimed he slipped off a bolt with a wrench and felt a twinge in his rt. shoulder. He continued working. He was seen by medical doctor on 9/21/07. Pain has persisted for this employee, and he is now diagnosed (11-08-07) with AC joint DJD and is being treated with injections of Cortizone. He has a lifting restriction until released from this treatment.
