## Narrative

While preparing to work on filtrate door, employees removed the manway aside. When ee pulled the manway open, a large piece of scale slid out of the opening. The scale was the size of the manway and sharp all around. The scale hit him on the top of his foot and it began to swell. He was placed on restrictive work duty because of the limitations of his foot being swollen.
