A roof fall occurred in the 3 south panel in the #1 entry in the intersection of crosscut #14. The fall measured approximately 20'x18'x6'. The area was supported with 5' gully grouted bolts and 9' mega bolts. An undetected slip was determined to be the cause of the fall.
