## Narrative

A roof fall was found at 2:39 pm at cross cut #5 entry 3 main west. This four way intersection had been mined Jan 1998 & was bolted with 8' full resin pins. Fall was 20' long x 20' wide x 10' high. No injury or property damage resulted. This fall may have to be cleaned up at a future date. If so, MSHA will be notified. Fall was reported because it is adjacent to an escapeway.
