## Narrative

The lifting lug was only welded in one spot of the 6' X 12' X 1/4"" plate steel. This caused the plate steel to be too weak and unbalanced, therefore, it fell on EE when the forklift tried to move it. EE should have welded both sides of the plate steel. The steel plate weighed approx 500-700 lbs, causing EE lower back trauma.
