## Narrative

A mud rig had been installed on tank 68 to drain caustic to the west unit sump.  While taking the mud-rig off the bottom of tank 68, his hand slipped off the wrench and hit the 2"" ball valve on the bottom of the tank.   Caustic was released, splashing under his face shield onto his face.  He was wearing mono-goggles and a face shield at the time of the incident.
