## Narrative

Employee bent a roof bolt installing it in the roof and as he was attempting to get it out of the roof the bolt head came out of the drill head chuck striking him on the wrist. Cause of this injury was safety awareness, compliance with rules/regs was not a factor, miner proficiency and attitude was a factor, protective equip was not a factor.
