## Narrative

Employee was holding a backup wrench while another contractor was using an impact wrench to tighten nut / bolt assembly.  While being turned with the impact wrench, a welded connection on the bolt broke causing the assembly to spin loose striking the injured employee on the left index finger.  No days away.  Medical only.
