Narrative

An unintentional roof fall occurred in the Main South at Entry #4 at +2625 south. The area was supported with 4' conventional bolts. The fall area is approximately 21'W x 20""L x 4'- 5' H. The strata broke just at the anchor point. An evaluation of the area was conducted.
