Narrative

Employee reported on 1/24/08 that he had got something in his eye on 1/23/08. When he woke up on 1/24/08 with redness and irritation and noticed something on his cornea. He was taken to the ophthalmologist who removed the foreign body and prescribed an antibiotic. The employee returned to work with no restrictions.
