Climbing down from access hatch approx 10' above landing and 50"" above walkway, employee slipped, scraped right lower back on metal step. Reacting to the injury, grabbed bar, swung down to walkway stepping on the railing horizontals, rolling to deck on his back. He was wearing double lanyard and harness but failed to use it in accordance with company policy.
