On 02/03/2008 an unintentional fall of roof occurred at cross cut #11 between #5 & 6 entries on the 1st West Sub Main off the 4th South Sub Main. The fall was approximately 84' long x 10-17' thick x 17' wide. The area was bolted with 6' and 10' point anchor bolts. The fall will be cleaned up and supported.
