## Narrative

Upon an investigation supplies was being loaded by a John Deere fork lift to be taken underground by the employee. The employee dismounted the diesel underground supply vehicle and stepped directly in front of the fork lift. He was struck in the legs by the fork and knocked down into the supply vehicle striking his head against the metal frame.
