## Narrative

EE was pounding the shell off a top broken middle roller with the belt running. Drill steel touched the moving belt & his arm was drug over the side middle roller. Cause of this injury was an unsafe act and not locking out the belt. Complaince of rules and regs, miner attitude a factor, mining equip. was a factor.
