Narrative

A roof fall occurred in the 1st South Mains, #1 entry, between 20+80 and 21+60. The fall measured 60'x 19'x 7' and was caused by an undetected slip in the mine roof. The area was mined in July 2006 and supported with 6' tension rebar roof bolts.
