## Narrative

While closing the red chain valve that directs water to different tanks, the door next to the work area had malfunctioned and tarps had been put in place. Due to the cold weather the floor had become very slick with ice build up. As he was closing the valve he lost footing and fell to the ground resulting in an upper left arm fracture
