## Narrative

Walking from a pump station and slipped on a 3"" poly-line used for suction. He fell and struck his left rib and the lower part of his left leg. He continued to work until 3/4/08. After receiving medical attention, it was determined he had a deep vein thrombosis. His first day off work was 3/5/08.
