EMPLOYEE ACTIVATED ROOF BOLTER CANOPY RAISE LEVER. AS IT ROSE, IT HUNG UP ON A BOLT AND THE STRAIN CAUSED THE CANOPY TO FALL, STRIKING EMPLOYEE'S RIGHT ELBOW.
