## Narrative

On 02/12/08 employee was fueling equipment when he slipped on the ice and fell with his left arm extended which resulted in a shoulder injury. He was taken to the doctor and returned to work with no restrictions. On 02/25/08 an MRI was done which showed a tear of the left rotator cuff. He missed his first day of work on 03/13/08 when surgery was performed to repair the tear.
