## Narrative

EE was roof bolting at the time of this incident.  He stated that he was in the process of drilling a hole for a 6 foot bolt, when a small rock fell from the roof and struck his drill pot.  The rock landed on the spinnig plate that holds the chuck.  This caused the rock to fly off the drill pot and strike him in the mouth, breaking a portion of his left front tooth.
