Employee got his foot piniched between a 4"" h-beam laying along shields and the toe of the shield when the Petitto mule bumped the beam. Cause of this injury was safety awarness. Compliance with rules/regs, protective equip., was not a factor, mining systems could be a factor, miner proficiency a factor.
