## Narrative

Employee was in process of cleaning up tools/materials at end of shift. He had unknowingly placed his foot between 4 bolts that secured a cap/lid on floor.  As he twisted to right to retrieve a tug-it laying on the ground, felt pain in left knee.  He states that his left boot was boot was wedged between the bolts.
