Narrative

Employee was roof bolting on unit 1. As he was drilling a hole a fitting broke off a trunion jack and sprayed out oil. As he tried to get out of the way he stumbled and struck his mouth on the drill pot. He suffered injuries to three fron teeth. He had to have one tooth extracted.
