A roof fall was discovered on the #3 main #9 entry #16xc in a 3-Way intersection. The fall measured 17' x 20' x 8 ft high. The area was bolted with cable bolts and 6 ft fully grouted rebar. The fall did not trap men or material or equipment and it did not affect ventilation or escapeway.
