Narrative

Employee's shift began at 6:00 p.m. on 3/5/2008. Employee was punching out picks using an air hammer. A piece of metal flew off the hammer head and lodged in the employee's leg. Employee had surgery on April 4, 2008 making this a reportable injury effective that date.
