Narrative

Injured was using an 8lb sledge hammer to strike 8"" feed pipe, when hammer glanced off bottom of pipe, causing hammer to drop down and pinch index finger between hammer handle and coarse coal screen. Resulted in a laceration to index finger, requiring 6 stitches.
