Narrative

He was climbing down from the D10 dozer and felt pain in his left lower back and shoulder. He continued to work at his regular duties, eventually went to a physician and was diagnosed with ""Advanced Degenerative disk disease"". He was removed from work on 4/24/2008 for surgery. Please consider this case as an Occupational Illness.
