Narrative

Employee was putting a bottom roller on a trailer when he strained his back. He continued to work and went for an MRI on the 31st of March 2008. The result of his MRI came back on the 2nd of April, it was positive. He scheduled his surgery for April 18th. The employee started missing work on the 15th of April.
