## Narrative

While working on #3 cyclone feed pump, employee  went to retrieve a tool from his tool locker. While walking  back through an area with mud, his foot stuck in the mud pulling his lower back muscles. Went to the doctor on April 24th, Missed work on 25th & 28th. Returned on the 29th of April.
