Narrative

Employee was n the process of extending line curtain when a section of roof fell measuring 7.5'L x 6'W x 0-14"" thick from the unbolted face and extending approximately 16"" past last row of bolts to where he was standing under a permanently supported area striking him across both legs resulting in fracture to left big toe and tailbone.
