Injured had scaled area and replaced a loose bolt and when he went back by bolter tray a piece of rock 55"" X 23"" X 7"" fell from the roof, striking him in the lower back and he fell, hitting his nose and upper lip on bolter tray. 5 stitched to upper lip.
