Narrative

Employee had finished one row of bolts. and was examining roof preparing for second row. When a piece of scale measuring 15""x13"". 0-3"" thick fell on employee causing a laceration to the back r/s on head. This resulted in 16 staples to close the wound, and neck. back soreness.
