Employee reported on 4/15/2008 that he had pain in his neck, right shoulder and right arm which he feels may be related to a 1996 mine injury in conjunction with his 30+ years of coal mining performing tasks requiring heavy lifting and carrying. Employee had neck surgery on 06/05/2008.
