Narrative

The employee was removing 3""pipe suspended from the mine roof by chain and placing it on a pipe trailer. He unhooked a section of the pipe and left the chain hanging from the mine roof, the end of the chain became wedged under the pipe. The employee grabbed the chain to free it from under the load and the chain released striking the employee in the mouth chipping a front tooth.
