## Narrative

Maintenance employee was attempting to position a new chute that was connected to a chainfall by way of a padeye. He applied force in a cross direction to align the chute and the weld failed on the padeye. The chainfall buckle and padeye came back and struck the employee in the forehead causing a laceration.
