## Narrative

It was dark and not well lighted. While enroute to the shift supervisor's office he stepped through a 6""-8"" gap in the grating covering the hole above row #3 cell #3 in the floatation area.  He caught himself with his Rt wrist and shoulder to keep himself from falling.
