Narrative

An unintentional fall occurred at the 6+30' in #4 entry in 2nd West sub main. Fall area was bolted with 6' tension rebar bolts and 10' cable bolts had also been installed as supplemental support. Fall was 20' long by 70' wide by 11' high.
